Financial Information
In an effort to keep dental fees down while maintaining a high level of professional care, we have established the following plans of payment for the use of our patients:
- Payment in full for each visit is due unless financial agreements have been made in advance.
- We will gladly accept payments from dental insurance companies. Current insurance information must be provided at each visit. The patient will be asked to pay the portion not covered by insurance at the time of the visit. The patient is responsible for any unpaid insurance claim after 45 days of the date of service.
- For major dental procedures, one-half of the total fee is to be paid when treatment is started and the balance is due when treatment is completed.
- We accept the following bankcards: VISA, MasterCard and Discover
- Returned checks will be charged the $20 fee our bank charges us.
- Divided Payment Plan: Interest free financial arrangements may be made through Norwest Financial for either 6 months (balances less than $1000.00) or 12 months (amounts greater than $1000.00). Applications are available through our office.
- Patients with a strong financial history with our office may be given the option of paying one-half at the start of treatment, with the balance payable in three equal payments over the following ninety days.
Insurance Claims Filing
As a courtesy, we will file claims to your insurance carrier for the primary insured for each visit to our office. We ask that you update all insurance information each time you come to the office or when it changes, whichever comes first.
At this time, we continue to accept benefits assignments made by the insurance carriers to our office for the treatments provided to the insured. Any claim not paid by the insurance carrier, for any reason, more than 30 days after it is filed is due and payable in full by the insured. An unpaid balance from any insurance claim is the responsibility of the insured and/or their guarantor.
Claim re-filing for incorrect insurance information:
Claims returned to us for incorrect information will be handled as follows:
- The procedures covered on the claim will be due and payable by the insured to our office. According to our insurance office policy, we cannot wait for the additional processing of the corrected insurance claims to be paid. We will print a new insurance claim form on paper that can be obtained by the insured for self-filing.
- When we have the corrected insurance information, we will, at the request of the insured, re-file the claim for an additional non-refundable/non-negotiable filing fee of $25.00. The amount of the claim is still due and must be paid by the insured before we re-file.
Insurance Benefits and Covered Procedures:
Our practice philosophy is one of patient centered care, not insurance directed care. It is most important to us that we correctly diagnose and recommend the most appropriate treatment at the highest standard of care to each of our patients. We then coordinate the proposed treatment plan to whatever insurance benefits may be available. While we completely recognize that dental insurance benefits are an important factor in the delivery of any care provided to our patients with dental insurance, we do not diagnose and provide oral health care based on insurance benefits.
Each dental insurance plan includes procedures that will be covered and the amount of benefit dollars allowable for the procedures. The covered procedures are a factor of the negotiation between the insured's employer and the insurance company. We do not have any input into what procedures are covered nor the amount of the insurance re-imbursement for any covered procedures.
Further, we do not know what procedures are covered in any given policy. The employer provides that information to each insured and as such, it is the responsibility of the insured to know what benefits are available. Such information is never provided to our office.
We attempt to try to estimate what insurance benefits may be available to each patient to better predict the out-of-pocket expense that may be expected after insurance benefits. Our estimates are not meant to be exact and cannot be interpreted as absolutes because we do not know for sure what insurance benefits are available in any given policy. Again, it is the responsibility of the insured to know their dental insurance benefits.
Coordination of dental insurance benefits can be confusing and difficult at times and we are very willing to act as an advocate to help our insured patients in securing all the benefits they are entitled to under their insurance policy.
Patients with more than one dental insurance policy:
Our office only files for the primary insurance policy. Any secondary, or in rare cases, tertiary dental insurance claim filings are the responsibility of the insured. We do not accept assignments for secondary or tertiary dental insurance policies. The insurance company will directly reimburse the insured, as our office will already have been paid.
